Children are not just small adults and their anatomy and physiology is different than in adults. A gentle approach is necessary while performing PCNL in children. Miniaturization of the instruments has been popularized especially within the last decade. The main advantages of miniaturization are; less bleeding, less trauma to the kidney, avoiding the use of nephrostomy tube postoperatively which results in early recovery and less hospital stay. On the other hand, 'smaller is better' idea does not work for every size and type of stone in children. Therefore, choosing the optimal tract size with an effective fragmentation is a state of art.
